Medical Social Work: The Advocates

Our Medical Social Workers assess the emotional, environmental, and economic conditions that affect a patient’s well-being — the pieces of the puzzle a nurse or therapist isn’t necessarily positioned to address. Their work includes comprehensive patient and family assessments, direct referrals to community resources, and consulting with the physician when social or financial barriers are affecting a patient’s health.

For families facing a difficult decision about nursing home or alternative institutional placement, our social workers also provide guidance and support through that process — along with ongoing rehabilitative and supportive case work as the plan of care evolves.

Home Health Aides: The Daily Support

While nurses and therapists handle the clinical side of care, our Certified Nursing Assistants (CNAs) step in for the day-to-day: bathing and grooming, oral hygiene, hair, nail, and skin care, safe feeding assistance, and simple, non-sterile dressing changes. They also help keep a patient’s home environment safe and orderly — the kind of steady, hands-on support that makes daily life manageable.
